I authorize Forza Construction Services to investigate my previous background in connection with its consideration of my application. Through this document, or a copy, I authorize any person, agency, institution, company or other entity to give Forza Construction Services any and all information they may have, or in connection with the information they receive. I further release from liability any individual, including, including records custodians, for damages that may result to me due to compliance or any attempts to comply with this authorization. I consent to undergo a physical and a drug test, as an applicant or employee, upon request and at the discretion of Forza Construction Services and consent to have the results of those test disclosed to Forza Construction Services. I understand my application for employment will be denied or my employment will be terminated, if upon request I refuse to submit to testing or refuse to consent to have the results disclosed or discussed, or, if there is a confirmed positive test result. If employed with Forza Construction Services, I understand that I would be obligated to abide by all rules, regulations, policies and procedures. In consideration of my employment, I further acknowledge and agree that my employment and compensation may be terminated, with or without cause, and with or without notice, at any time at the option of either myself or Forza Construction Services. ‍

Consiento en experimentar un fisico y una prueba de la droga, como un solicitante o el empleado, sobre el pedido y a voluntad de Forza Construction Services consentimiento para tener los resultados de esos pruban revelado a Forza Construction Services. En consideracion a mi empleo, yo reconozco aun mas y concuerdo que my empleo y la compensacion se pueden terminar, con o sin la causa, y con o sin nota, en tiempo en la opcion de o yo mismo o Forza Construction Services.
